What is Gazelle?

Gazelle® is the mobile health app from Quest Diagnostics that empowers you to see, store, and share your vital health information, and take control of your health anytime, anywhere.

What are the benefits of using this app?

What other medical information can I store?

Gazelle allows you to store your doctor, insurance and emergency details, allergies, problem list, vital statistics, prescriptions, lab results and medical and immunization histories.

Why is Quest Diagnostics sending results directly to my patients?

Currently, over 1 million times each year, Quest Diagnostics is sharing results with patients in the states where it is legal. In thirty-seven states, laws do not prevent patients from having access to their medical records, including laboratory test results. Gazelle simply allows us to provide results electronically to the patient.

What other services will be made available to my patients?

In addition to receiving lab results on Gazelle, patients will also be able to use the app to see, store and share:

  • Prescriptions
  • Physician Information
  • Immunizations
  • Emergency Contacts
  • Medical Conditions
  • Allergies

Who notifies the patient when a result has been amended?

If a result is amended, the physician will be notified by Quest Diagnostics. The patient’s physician is responsible for communicating amendments to the patient.

Using Gazelle

How do I get started?

Getting started is easy:

  1. Using your mobile device, visit GazelleApp.com and download the app.
  2. Using your mobile device, open the app and:
    1. Enter your Quest Diagnostics Patient Account user name and password and follow the instructions to begin entering your health information.
    2. If you don’t have a Quest Diagnostics Patient Account, click on the ‘Create an Account Now’ link and the app will walk you through creating your Quest Diagnostics Patient Account and entering your health information.

What is a Quest Diagnostics Patient Account?

A Quest Diagnostics Patient account is a secure account in which your demographic information is saved reducing the time it takes you to conduct future transactions such as scheduling appointments and paying your Quest Diagnostics bill online.

I don’t have a Quest Diagnostics Patient Account. How do I create one?

The first time you open the app we will walk you through how to create a Quest Diagnostics Patient Account. You will need to create a user name and password, answer a few security questions and enter some basic demographic information.

I created my account, but I never received an email to validate my email address, what do I do?

The first place to look for the missing verification email is your spam folder. If you still cannot find it, let us help. Give us a call at (877) 291-7521 or send an email to GazelleSupport@QuestDiagnostics.com.

How do I share my information?

Using the ‘Send Health information’ icon you can email or fax your health information to anyone you choose.

How can someone access my information in the event of an emergency?

Medical responders often search for contact and medical information by looking in a patient’s wallet or mobile phone. The Gazelle website provides a card that you can print and keep in your wallet with instructions for accessing emergency information. The card will notify emergency medical responders to find your important health information on your mobile device.

How it works:

All of your medical information is secure and only accessible with a password; however, your emergency information is accessible without a password.

Emergency use information is commonly referred to as “ICE” (In Case of Emergency) and includes:

    • Your name
    • Your primary emergency contact and primary care physician
    • Medications
    • Medical conditions, and
    • Allergies

If I delete the application from my phone do I lose my information?

No, we continually synchronize the information on your mobile device with our secure servers in case you need to recover your information.

What happens if I lose my phone and/or get a new phone?

No problem, we can synchronize the information on your new mobile device with our secure servers in case you need to recover your information.

How do I enter my medical information?

It’s easy to enter your health information. The first time you use Gazelle, “Quick Start” will walk you through entering your most critical health information. You can skip “Quick Start” or bypass any section(s) and enter the data later.

Can I get results via Gazelle for any test that Quest Diagnostics performs?

Most lab test results can be requested via Gazelle. However, some tests are very complex and for those tests, your physician will deliver the results to you. If the test results you are requesting fall within this category, we will notify you via email.

How long does it take to get my results?

Most tests are performed within just a few days. Some complex testing can take a few weeks to complete. Results are sent directly to your healthcare professional as soon as they are ready to enable him/her to review and share the results with you directly. Then if requested, lab test results will be sent directly to your Gazelle app 2 business days later. If you have requested your results be sent to your Gazelle app, you will receive an email when your results have been released to you – usually within 3-4 days of the request depending on the test performed and the state you live in. Some states require us to hold results for 7 or more days.

How will I be notified that my results are available?

You will be sent an email informing you that your results are available.

How far in the past can I request results?

Results for tests performed within the past 2 months are available through Gazelle.

How much of my health information can be saved?

There is no limit to the amount of non-lab data or lab test results that can be stored.

Can I include health information for my family?

Currently, Gazelle stores information for one person with the exception of vaccinations. With Gazelle, you can include vaccinations for your others like your children within your app.

My doctor, pharmacy, hospital and insurance information are already in the contacts section of my mobile phone. When entering my information in Gazelle, must I type this information in manually to populate the data or can the application automatically import the information on my contacts?

Gazelle provides the ability to import this information automatically. Choose “Add” , then select the “Contacts” button

How do I add additional data for allergies beyond what’s listed?

There is an option to enter “My Other Allergy” in the Medical Information section of the application. In fact, many of the sections provide the ability to enter additional information.

Do I need to have my blood drawn at a Patient Service Center (PSC) to get my results sent directly to me?

No, Quest Diagnostics will send you results no matter where you specimen was collected, as long as your specimen was sent to Quest Diagnostics to be tested.

What if I do not live in a state that is listed for this offer, how can I get my results?

Quest Diagnostics can provide your Quest Diagnostics lab results directly to you, with the Gazelle app if you live in an eligible state. If you live in Puerto Rico or another state you can still receive your results:

  1. Online
    If your physician is using the Quest Diagnostics Care360 system, and
    1. Your physician agrees to release your results, and
    2. You have a linked account with Gazelle or Microsoft® HealthVault™.

  2. Directly from your physician
    1. Next time your physician orders a lab test, ask your physician to indicate on the requisition that Quest Diagnostics should also send you a copy of the test results. Most tests take between 1-10 days to complete.
    2. If you want a copy of previous test results you can ask your physician to send you a copy.
  3. By requesting and filling out a form to get your test results mailed to you directly from Quest Diagnostics if your state of residence permits us to release them. See state availability. Once we receive your completed form, you will receive your test result within approximately 30 days.

How do I request an “Access Form” if my state permits Quest Diagnostics to release results to me?

You can send an email to the Privacy Officer at Privacy@QuestDiagnostics.com to request a form or call the regional laboratory closest to you. Once we receive your completed form, you will receive your test results within approximately 30 days.

Do I need to have a PHR from Microsoft or Quest Diagnostics’ Care360 to download lab results to Gazelle?

No, Gazelle does not require you to have an account with an external PHR.

What if I scheduled my reminder on my Gazelle app on my Blackberry and I have Gazelle on my iPad as well, will I get the reminder on both my Blackberry and iPad?

Yes, Gazelle will remind you on whatever device it is loaded on provided you have logged in onto that device after you set up the reminder on the other device. Gazelle will synchronize the reminders on each device during login.

I didn’t get my reminder after I set it up. What should I do?

In order to get your reminder, check the following:

  • Your phone is turned on
  • The reminder is turned on
  • The reminder start date is not in the future
  • The reminder end date has not passed

What if I forget to take my medication when I was supposed to, what should I do?

Gazelle empowers you to set up and follow your medication schedule easily and conveniently but you should not rely on Gazelle reminders alone to take your medications. If you have any questions about your medications and how they should be taken, contact your physician.

My BlackBerry is the property of my employer and data is backed up on a server on a regular basis. If I email my lab results, will these lab results reside on our backup server and be visible to the IT staff?

Maybe. If you are using your company’s email address to email your lab results, depending on your company’s retention policy your information may reside on your company’s email server. Please check with your company.

Can I share selective information with others? Can I elect this option using the Gazelle application?

Gazelle provides several options to share information including email or fax. You may share your full medical profile or just your emergency medical information.

What would happen if I had a second mobile device and desired to gain access to Gazelle?

Just download the app to the other device and log in with the same username and password.

Would I have to reenter my data to use it on a second device?

No. Your data is backed up on a Quest Diagnostics server. If you get a new mobile device or have multiple devices there is no need to reenter your data.

Will using this application increase my data usage fee?

This application does involve data usage and you will be charged according to the data plan you have with your mobile carrier — a one time download of the application (approximately 600KB) and ongoing synchronization.

Is my information secure?

We implement industry best practices to protect your information:

  • You are required to use a username and password to access the app
  • We use secure HTTPS encryption over the air as we transmit your data to our servers
  • Our servers comply with all regulatory rules associated with protecting personal health information

I created a user ID and password however I am only asked for the password, why?

If you check the “Remember Me” box on the login screen, the application will not ask for a user ID to save time and keystrokes on subsequent use.

What are the terms and conditions of using this app?

If you use Quest Diagnostics’ mobile services to enter and maintain your personal information, you understand that you are responsible for safeguarding and securing your mobile device. If you leave your mobile device unattended, or if it is lost or stolen, you understand that your personal information may be accessible to others.

Why do I have to confirm who I am?

Regulation and contract require us to only release lab results to authorized people. We have chosen what is called Knowledge Based Authentication (KBA) to verify you are who you say you are to a high degree of certainty, without actually seeing you. Knowledge Based Authentication is the current best practice used by financial houses, insurance companies, credit bureaus and retail merchants to verify who customers are when they are not present. If you’re an adult who’s 18 or older, you will be required to use online identity verification before you can request laboratory results through Gazelle.

How does this Identity Verification process work?

The first time you select Request Results from within Gazelle, you will be asked to authorize us to send your results to your Gazelle app. Only then will you be asked to confirm your identify. After this is done, you will be able to request your laboratory results. Here’s how it works:

  • We securely send your name, date of birth, and address (not your Health/Medical Record information) to a company we contract with to provide this identity verification service.
  • This company securely sends us a series of multiple-choice questions that are based on your personal information. When you submit your answers, we securely transmit the questions and answers back to the company, which checks your answers against the information from public records.
  • Then, the company securely sends a message telling us whether you answered your questions correctly.
  • If you answer these security questions correctly, you can start requesting your laboratory results through Gazelle.

If I change my personal information, do I have to verify who I am again?

Gazelle will prompt you to confirm who you are the first time you request results. After that, anytime your name, date of birth, or address changes in Gazelle, we need to re-verify your information to ensure you are the person requesting results.

Gazelle said I could not be located – what am I supposed to do?

You should double check your personal information, including name, address, and date of birth – to ensure it is in Gazelle correctly. We often find that patients who have recently moved are not able to be found because public records have not caught up with their most recent information.

Some people may not have enough information available in public databases to confirm their identities. For example, if you are a foreign citizen, recently changed your name or are a young adult without very much public history, you may not be able to use this feature. If our third-party contractor cannot find enough public information to confirm your identity online, we will be unable to send your lab results through Gazelle.

Why don’t the Identity Verification questions I am asked pertain to me?

If you have recently moved or changed your name, it is possible that your information is not updated in public records and as a result the questions you are asked pertain to someone else. Double check your personal information in Gazelle to be sure it is correct. If it is, and you are still unable to verify your identity, then wait a few months and try again.

I answered some Identity Verification questions wrong and I am locked out. What should I do?

To protect your identity, we lock out users who cannot answer the questions in the first round. We currently cannot manually unlock you, so you will have to wait either 36 or 72 hours depending on the reason you were locked out and try again.

How is my information transmitted and stored?

To protect your information, all questions and answers used to confirm who you are, from Gazelle and those from the third-party company accessing your public data, are encrypted (or translated into an unreadable code) so that unauthorized users can’t read them. This information is not stored in our databases or computer systems; it is only used to confirm who you are so you can request lab test results.

The company confirming your identity is required to use strict privacy and security protections. They may not use the information Quest Diagnostics sends for any other purpose other than confirm who you are so that you can receive your lab results through Gazelle.

Requesting Lab Results

Can I get results in Gazelle?

Quest Diagnostics can provide most lab results through Gazelle if they were performed by Quest Diagnostics and you live in an eligible state. We are not currently delivering the following results through Gazelle,

  • Most lab test results can be requested via Gazelle. However, some tests are very complex and for those tests, your physician will deliver the results to you. If the test results you are requesting fall within this category, we will notify you via email
  • Pre Employment / Employer sponsored Drug Testing, please contact the employer for these results.

If my state permits the release of lab results, but they are not available through Gazelle, how can I get them?

You can request and fill out a form to get your test results mailed to you directly from Quest Diagnostics if your state of residence permits us to release them(See state availability). Once we receive your completed form, you will receive your test result within approximately 30 days.

Can I get results for my child through Gazelle?

Gazelle is available for everyone, but lab results delivery through Gazelle is currently only available to Quest Diagnostics patients 18 years and older.

How do I request results in Gazelle?

Getting your lab results in Gazelle is simple. All you need to do is:

  • Sign into Gazelle with your existing Quest Diagnostics User Name
  • Select “Lab Results”
  • From “Lab Results”, select “Request Results”
  • Fill in the information requested for the lab results you want and click “Submit”

You will receive an email confirming we have received your request and you will be notified when the results are available for you to view on your smartphone.

How long does it take to get my results?

Most tests are performed within just a few days. Some complex testing can take a few weeks to complete. Results are sent directly to your healthcare professional as soon as they are ready to enable him/her to review and share the results with you directly. Then if requested, results are sent directly to your Gazelle app 2 business days later.     1. Request your physician provide you with a PIN from the physicians Care360 system
    2. Enter the PIN in “Connect with Physician” menu item in Gazelle. This is found under the “Request Results” main menu item
    3. Request the physician selects to release your results (called “send to e-patient”) from their Care360 system

  2. Directly from your physician
